Care Coordination supports people with developmental disabilities in identifying and obtaining supports and services through person-centered choice and self-determination. Service Coordination also provides oversight for any health and safety issues.
RCAL’s Care Coordinators are experts on community services, resources and entitlement programs. They also advocate for access to programs and services under the direction of the individual and are directly accountable to individuals and their families.
Care Coordinators are independent to ensure a conflict-free environment and that they only refer people to services/supports that are appropriate. RCAL has a strongly person-centered approach in which the wishes of the individual are most important. For this reason, we encourage potential consumers or families to interview our Care Coordinators before selecting an RCAL Service Coordinator.
Care Coordinators can help to refer and link individuals to the following services:
- Waiver Services like Respite and Community Habilitation
- Housing and Housing subsidies
- Employment
- Medical/Clinical Services
- Counseling
- Special Equipment and Environmental Modifications
- Transportation
- Recreation
- Benefits and Entitlements
- Other resources as requested, based on individual interests
